When a key member of staff calls in sick at the last minute, Sister Rachael's dream of a smooth shift disappears. Instead, she has to manage a department overflowing with patients, and struggle with staff shortages, meaning longer waiting times, and no available beds. After having fallen down the stairs the night before, 44-year-old Gavin woke up with a sore neck. He walked himself into A&E, but was immediately put into a neck brace and rushed into radiology. The results shocked even the most experienced consultant treating him. It's Betty's 90th birthday, but rather than celebrate it at home, she's cut her finger and it won't stop bleeding. And two-year-old Lottie has fallen over at nursery, leaving mum Emma convinced that something sharp is lodged in her foot — will the doctor find anything? And 62-year-old Sandra suffers from a longterm, excruciating condition that means she's been into A&E more than 40 times. Dr Reddy admits that he and his colleagues sometimes feel helpless not being able to do more for those patients who have chronic ailments. Over in resus, 51-year-old Claire has had an accident while out walking her two dogs. The elbow is one of the most painful joints to dislocate, but hers is more complicated than usual. The doctors need to quickly discover the best method of getting it back in place, without causing her even more pain.

The next emergency is assigned to Dr Chris, who's being shadowed by junior doctor Sophie. They're treating 55-year-old Tracey in the high-dependency unit after she was rushed in with severe breathing problems. Tracey had a double lung transplant seven years ago, so it's vital the team get to the bottom of her condition before she deteriorates any further. And in the assessment hub, still battling a full waiting room, Sister Jane examines 75-yearold John's hand after a fall. His finger is badly damaged and needs an X-ray to determine if it needs surgery. But first, his ring, a family heirloom, needs to be cut off.

An emergency delivery of ice pops provides temporary relief, but the heat's taking its toll on the hospital generator. Deputy Medical Director Jo rushes from ward to ward, turning off the lights in a desperate attempt to stop it shutting down. Meanwhile Vicky faces staff shortages - and the water cooler's thrown a wobbly. As the hub gets hotter, the emergencies keep coming. Fifty-year-old Michael's brought in after collapsing at home. His family are worried he's had a seizure, but after examining his blood tests, Dr Blanchard has another theory. It's quite a day for hand injuries. Lead cutter Stephen's sliced open three fingers on his circular saw. Emergency Nurse Practitioner Jo administers pain relief and tells him he's lucky - he could have lost his fingers. Meanwhile, refrigeration engineer Mark is admitted after breaking his wrist at work. Waiting to hear if he'll need surgery, he rues his misfortune - he was only four weeks away from retirement. Over in Paediatrics, Abbie's in pain after hurting her hand on a fire extinguisher at school. Dr Klu's trying to treat her, but it's proving tricky - Abbie's afraid of needles.

Sister Rachel is in charge today and as a result of COVID she's short on space and staff. Every patient needs swabbing, and waiting times are long. But there's not just COVID patients to worry about - there's a raft of other emergencies too. Fifty-year-old Richard has rushed in with severe burns across one side of his body. He was setting fire to some rubbish with petrol when his clothes caught fire. He's in severe pain and the team needs to act fast to prevent infection and minimise scarring. Also in resus is 64-year-old Jan. His smart watch sounded the alarm when his heart rate dropped to a dangerously low level. Physician Associate Wayne needs to urgently raise his heart rate as his organs are struggling and could start to fail. In minors, 77-year-old Avril has arrived with a gash to the back of her leg. She was out shopping when a boxed BBQ fell onto her from another shopper's trolley, tearing her skin. It's a nasty cut but because her skin is so thin, stitching isn't an option, so Consultant James Griffiths has to find another way to treat the injury. Volunteer Jane is back from her holiday in Egypt and is catching up with colleagues she hasn't seen in a while. Her first job of the day is to clear the corridors of clutter that has amassed in her absence - and she's determined it will get done quickly. Lorry Driver Carl is brought in by ambulance after falling from the back of his wagon. He's not just fractured his wrist, the bone has broken through the skin, making things much more complicated. He's worried that a long-planned family holiday to Florida might now be in jeopardy.

In Resus, staff nurse Amy takes charge of his care, but Wojcieches drifts in and out of consciousness. Too little recovery time between seizures could lead to permanent brain damage.
Dr Chris Yeoman is urgently called over to Resus where 64-year-old Lynn has severe abdominal pains that he suspects are caused by a ruptured appendix. If confirmed, Lynn will need an emergency appendectomy.
In Paediatrics, Junior Doctor Josh Gemmill faces a different sort of challenge. Fifteen-year-old Thomas has a ring stuck on his injured finger that's threatening to cut off his blood supply. When the hospital cutting equipment isn't strong enough to remove the ring, Dr Gemmill has no choice but to call in the Fire Brigade.
And staff nurse Mimi plays a vital role today in reducing queues in the waiting room. She runs Triage caring for patients like 22-year-old Elliot who was attacked by a street gang just hours after being involved in a car accident.

It's mid-afternoon at Barnsley Casualty and the department is already gridlocked. Sister Jane urgently needs to move patients out of A&E, as more continue to arrive. To add to the pressure there's a shortage of porters, whose role is crucial when it comes to patient flow.
An 87-year-old, Ralph, who is struggling to breathe, is blue lighted in. Doctor Taylor prepares Resus with student Doctor Ewan. Ralph has come in with a complex set of health issues and Doctor Taylor needs to run a series of urgent blood tests, so that he can determine the cause of Ralph's critical condition.
Karina walks in with husband Darren after dislocating her shoulder for the fifth time. She's only just got over a dislocation in the same place after falling down a flight of stairs on holiday. Repeated dislocations could mean there's something seriously wrong, so Emergency Nurse Practitioner Darren needs to work quickly to try and relocate the joint.
Sallie, an 83-year-old, is rushed in after having a fall at home. Sallie's sent to cubicle 8 with her grandson's girlfriend Katie. She's recently had numerous falls, which could be the sign of a more serious underlying condition, so Doctor Amber Mitchell comes straight to assess her.
In Cubicle 7, Doctor Phil Taylor assesses Pui, who has back pain. He examines her to feel if there is any swelling of her kidneys. She's in agony and Doctor Taylor suspects she may have a kidney stone, so orders an urgent CT scan.
Over in Paediatrics, Nurse Ellie collects the next patient from reception. Harry has come with his mum Rebecca after an incident at nursery has left him with a large laceration inside his mouth.

In Paediatrics, an ambulance rushes in with five-year-old Ruby, who's suffered multiple epileptic seizures and has a high temperature.

Sister Jane has started her shift and has just walked onto the floor to find that every single area of the department is full, with 90 patients in need of help from the casualty team.
As if it wasn't busy enough inside the department, there's now a problem outside too. Doctor Jake Mullen and a team of nurses have been alerted to an 86-year-old screaming in pain in the car park. Mary has fallen over landing badly on her hip and once she's placed onto a trolley is rushed straight to resus. Doctor Mullen must work quickly to find out whether her injuries could be life threatening.
Another emergency is on the way. Paramedics are blue-lighting in a 54-year-old who's had back-to back seizures at home. The last time this happened abscesses were found on Paul's brain. Although Paul now appears well, there are too many red flags. Nurse Aleena immediately asks Doctor Hridesh Chatha to assess him. If the abscesses have come back, Paul is in danger of needing emergency brain surgery.
On busy days like today, the team of volunteers like Jane, play a vital role in keeping the department moving. One of the most vital jobs the volunteers help with is keeping the patients who are staying the longest, fed and watered.
Over in Paediatrics, four-year-old Kian may have broken his thumb after a fall. His mum Becky is with him and waiting to find out what the damage is. Nurse Greg assesses him and orders an urgent x-ray.
And paramedics are bringing in an 80-year-old who's been struggling to breathe. Advanced Clinical Practitioner Chantelle heads to Cubicle 4. Patricia's oxygen levels are worryingly low, so she needs to quickly find the reason why.

It's Bank Holiday weekend, but for the team at Barnsley Casualty it's business as usual. They're bracing themselves for a chaotic day and in charge of the shift is Sister Jane alongside Consultant James Griffiths. To make matters worse Barnsley are playing a key football match and once it's over they expect the patient numbers to suddenly rise.
The first emergency is on its way: 52-year-old Maggie is being rushed in after a serious car crash. Doctor Griffiths started treating this patient before he even clocked on, as the accident happened near to where he lives. Maggie has the bone disease osteoporosis, and it's suspected that she's badly broken her leg.
Over in Minors is the latest addition to the Barnsley Casualty team: Emergency Nurse Practitioner Clare. Her next patient is 70-year-old Stephen, who's suffered a deep wound to his head after falling over while walking his neighbour's dog. Emergency Nurse Practitioner Clare needs to remove every piece of grit embedded in his skin to avoid the wound becoming infected.
Back in the Hub, Sister Jane is trying to coordinate a crowded department, while Doctor Griffiths is wading through a sea of patients. His next is 26-year-old Jordan who's hurt his hand while riding a motocross bike. If he has broken a bone, it could have a major effect on his livelihood.
In paediatrics is another casualty of a sunny weekend. Nine-year-old Harrison has come in with dad Nathan, after being badly sunburnt from a day at the beach. Advanced Nurse Practitioner Liam is on hand to look after him.
