Severe weather conditions place the casualty department under even more pressure than usual. Dr Julian and his sidekick Sister Benita treat a pensioner with a broken ankle and a former miner with a chronic lung condition. Meanwhile, Dr Jane treats four-year-old Teddy after a sledging accident.

Staffing problems hit the department as several nurses call in sick. The remaining team are forced to juggle extra patients to make sure every emergency is dealt with. The pressure takes its toll on Sister Benita, and she is herself admitted to a bay for treatment.

Patient numbers are through the roof, and the team are facing one of the busiest days in the A&E's history. Keeping the team going through this incredibly pressured shift is the self-named Emergency Clown Sister Jane, who takes the younger nurses under her wing and boosts morale with a dose of laughter.

Meanwhile, doctor Bharath Reddy treats Maxine, who broke her wrist when she slipped de-icing the car, and another patient is treated after fracturing her shoulder.

This time, paediatric sister Jemma Wray treats two critically ill children - ten-year-old Kai has been having seizures, and 18-month-old Joel is struggling to breathe. Elsewhere, nurse Will Gibbons looks after a 28-year-old patient who has been in a motorbike accident, and 51-year-old Andrew comes into the hospital after damaging his fingers with a circular saw.

Instead, she has to manage a department overflowing with patients, and struggle with staff shortages, meaning longer waiting times, and no available beds. After having fallen down the stairs the night before, 44-year-old Gavin woke up with a sore neck. He walked himself into A&E, but was immediately put into a neck brace and rushed into radiology. The results shocked even the most experienced consultant treating him. It's Betty's 90th birthday, but rather than celebrate it at home, she's cut her finger and it won't stop bleeding. And two-year-old Lottie has fallen over at nursery, leaving mum Emma convinced that something sharp is lodged in her foot — will the doctor find anything? And 62-year-old Sandra suffers from a longterm, excruciating condition that means she's been into A&E more than 40 times. Dr Reddy admits that he and his colleagues sometimes feel helpless not being able to do more for those patients who have chronic ailments. Over in resus, 51-year-old Claire has had an accident while out walking her two dogs. The elbow is one of the most painful joints to dislocate, but hers is more complicated than usual. The doctors need to quickly discover the best method of getting it back in place, without causing her even more pain.

Every bed in the department is occupied, but there are still 20 patients waiting for the next one to be vacated. Lead Nurse Steph is already struggling to manage the mounting number of patients when the emergency alarm sounds and all available staff must head to resus to help a fitting patient.

They're treating 55-year-old Tracey in the high-dependency unit after she was rushed in with severe breathing problems. Tracey had a double lung transplant seven years ago, so it's vital the team get to the bottom of her condition before she deteriorates any further. And in the assessment hub, still battling a full waiting room, Sister Jane examines 75-yearold John's hand after a fall. His finger is badly damaged and needs an X-ray to determine if it needs surgery. But first, his ring, a family heirloom, needs to be cut off.

A 17-year-old arrives after falling from his bike and suffering a fit. Consultant Liz needs to find out if a broken neck is the cause of his arm numbness. In the hub, Sister Jane is struggling to manage the high volume of patients, not just those arriving by ambulance, but walk-ins too. One such walk-in is Johnathan, a 41-year-old recovering addict, who has infected ulcers on both legs.

It's another busy day for the casualty team with patient numbers unusually high. Sister Rachel is running the shift, and she's under pressure with a shortage of cubicles - to make matters worse, IT systems start to fail across the hospital, causing chaos for patient flow. Urgent treatment is needed for 75-year-old Janet, who's been viciously attacked by a dog and has a large flap of skin hanging from her elbow.

A former miner is brought in as he is struggling to breathe, while Sister Jane is battling a seemingly never-ending stream of emergency patients in the hub. Eighty-four-year-old Anthony has a suspected broken shoulder but, as he is also the carer of his vulnerable wife, Clinical Practitioner Chantelle needs to balance what is best for her patient with his caring duties at home.

There are no available beds in Barnsley Hospital's A&E department, while lead nurse Kelly is struggling with a backlog of ambulances waiting with patients needing treatment. In Resus, 86-year-old Grahame has been rushed in with severe chest pains and blood in his mucus and after assessing him, Dr Staunton needs a conversation with him - in private.

Advanced clinical practitioner Chantelle orders an urgent CT scan to asssess whether 79-year-old John has sustained any brain damage after falling at home and cracking his head. To complicate matters, John has a large cancerous growth on his cheek that has also been caught during the fall.

It is Hannah's first day in her new role of matron, but the department is completely full, there's no movement and patients just keep on arriving.

Sixty-five-year-old Theresa arrives by ambulance and paramedics suspect she has had a stroke, but her condition suddenly deteriorates and it's up to physician associate Wayne and Dr Laura to get to the bottom of her condition quickly.

A woman who is 18-weeks pregnant collapses in the toilets and is rushed straight to resus, where registrar Eamonn urgently needs to work out if she is at risk from a clot on her lung or a problem with the muscle in her heart. Advanced clinical practitioner Chantelle treats a 91-year-old woman who had a fall at home and has a nasty cut to her head.

It's the hottest day of the year at Barnsley Casualty. Sister Vicky has the job of keeping patients and staff cool, but with temperatures outside approaching forty degrees, it won't be easy. Volunteer Jane arrives to dispense drinks ('this could be the day when they need extra hands'). An emergency delivery of ice pops provides temporary relief, but the heat's taking its toll on the hospital generator. Deputy Medical Director Jo rushes from ward to ward, turning off the lights in a desperate attempt to stop it shutting down. Meanwhile Vicky faces staff shortages - and the water cooler's thrown a wobbly. As the hub gets hotter, the emergencies keep coming. Fifty-year-old Michael's brought in after collapsing at home. His family are worried he's had a seizure, but after examining his blood tests, Dr Blanchard has another theory. It's quite a day for hand injuries. Lead cutter Stephen's sliced open three fingers on his circular saw. Emergency Nurse Practitioner Jo administers pain relief and tells him he's lucky - he could have lost his fingers. Meanwhile, refrigeration engineer Mark is admitted after breaking his wrist at work. Waiting to hear if he'll need surgery, he rues his misfortune - he was only four weeks away from retirement. Over in Paediatrics, Abbie's in pain after hurting her hand on a fire extinguisher at school. Dr Klu's trying to treat her, but it's proving tricky - Abbie's afraid of needles.

There's a COVID spike at Barnsley Casualty and positive cases are flooding in, adding pressure to an already busy day. Sister Rachel is in charge today and as a result of COVID she's short on space and staff. Every patient needs swabbing, and waiting times are long. But there's not just COVID patients to worry about - there's a raft of other emergencies too. Fifty-year-old Richard has rushed in with severe burns across one side of his body. He was setting fire to some rubbish with petrol when his clothes caught fire. He's in severe pain and the team needs to act fast to prevent infection and minimise scarring. Also in resus is 64-year-old Jan. His smart watch sounded the alarm when his heart rate dropped to a dangerously low level. Physician Associate Wayne needs to urgently raise his heart rate as his organs are struggling and could start to fail. In minors, 77-year-old Avril has arrived with a gash to the back of her leg. She was out shopping when a boxed BBQ fell onto her from another shopper's trolley, tearing her skin. It's a nasty cut but because her skin is so thin, stitching isn't an option, so Consultant James Griffiths has to find another way to treat the injury. Volunteer Jane is back from her holiday in Egypt and is catching up with colleagues she hasn't seen in a while. Her first job of the day is to clear the corridors of clutter that has amassed in her absence - and she's determined it will get done quickly. Lorry Driver Carl is brought in by ambulance after falling from the back of his wagon. He's not just fractured his wrist, the bone has broken through the skin, making things much more complicated. He's worried that a long-planned family holiday to Florida might now be in jeopardy.

Patients are flooding into Barnsley A&E and a woman who has come in with a stomach ache goes into labour - and she had no idea she was pregnant. Next to arrive by ambulance is 87-year-old Ken, who had a fall at home and has a nasty cut to his forehead and cheek, so student doctor Saba Azizi stitches his cut while nurse Amy treats the wound on his cheek.

A patient is rushed into hospital suffering from serious burns, and he needs to be doused with water to try and stop the burns spreading further on his body. Meanwhile, a 74-year-old woman needs an emergency procedure after having a fall at home, and a nine-year-old girl has an X-Ray on her hand to check for any signs of a break.

The waiting room in Barnsley Casualty is overflowing today: Resus is crowded and there are no spare beds in the hospital. Sister Jane, who's running the hub, will have to juggle patients to make room for new emergencies.

Fifty-two-year-old warehouse worker Wojcieches is rushed in by ambulance after suffering an epileptic seizure at work. In Resus, staff nurse Amy takes charge of his care, but Wojcieches drifts in and out of consciousness. Too little recovery time between seizures could lead to permanent brain damage.

Dr Chris Yeoman is urgently called over to Resus where 64-year-old Lynn has severe abdominal pains that he suspects are caused by a ruptured appendix. If confirmed, Lynn will need an emergency appendectomy.

In Paediatrics, Junior Doctor Josh Gemmill faces a different sort of challenge. Fifteen-year-old Thomas has a ring stuck on his injured finger that's threatening to cut off his blood supply. When the hospital cutting equipment isn't strong enough to remove the ring, Dr Gemmill has no choice but to call in the Fire Brigade.

And staff nurse Mimi plays a vital role today in reducing queues in the waiting room. She runs Triage caring for patients like 22-year-old Elliot who was attacked by a street gang just hours after being involved in a car accident.

It's mid-afternoon at Barnsley Casualty and the department is already gridlocked. Sister Jane urgently needs to move patients out of A&E, as more continue to arrive. To add to the pressure there's a shortage of porters, whose role is crucial when it comes to patient flow.

An 87-year-old, Ralph, who is struggling to breathe, is blue lighted in. Doctor Taylor prepares Resus with student Doctor Ewan. Ralph has come in with a complex set of health issues and Doctor Taylor needs to run a series of urgent blood tests, so that he can determine the cause of Ralph's critical condition.

Karina walks in with husband Darren after dislocating her shoulder for the fifth time. She's only just got over a dislocation in the same place after falling down a flight of stairs on holiday. Repeated dislocations could mean there's something seriously wrong, so Emergency Nurse Practitioner Darren needs to work quickly to try and relocate the joint.

Sallie, an 83-year-old, is rushed in after having a fall at home. Sallie's sent to cubicle 8 with her grandson's girlfriend Katie. She's recently had numerous falls, which could be the sign of a more serious underlying condition, so Doctor Amber Mitchell comes straight to assess her.

In Cubicle 7, Doctor Phil Taylor assesses Pui, who has back pain. He examines her to feel if there is any swelling of her kidneys. She's in agony and Doctor Taylor suspects she may have a kidney stone, so orders an urgent CT scan.

Over in Paediatrics, Nurse Ellie collects the next patient from reception. Harry has come with his mum Rebecca after an incident at nursery has left him with a large laceration inside his mouth.

A patient is rushed in after a suspected overdose, unconscious and with a slow heart rate, while on the flipside, a 58-year-old woman is blue lighted in with a dangerously fast heart rate. In the paediatric department, an 11-year-old comes in with his mother and brother after hurting his head at school.

The IT systems have failed, causing chaos, but Casualty still need to treat a motorcycle courier who's had a head-on collision, and a man with severe abdominal pains.

After being knocked off his motorbike on his way to work, 62-year-old Peter is brought and is taken straight into resus, where Dr Chris Yeoman takes the lead on his care. Peter's blood pressure is dangerously low and he's also badly damaged his ankle so he's taken for an urgent CT scan. In Paediatrics, an ambulance rushes in with five-year-old Ruby, who's suffered multiple epileptic seizures and has a high temperature.

It's a hectic start to the afternoon shift for Sister Benita, who's running the hub on her first week back from maternity leave. An ambulance arrives with 75-year-old Barbara, who was rushed in after collapsing in her garden, while Dr Josh Gemmill tends to 78-year-old former miner David, who has a large haematoma on his arm.

Amna, 24, fell face first after collapsing at work and paramedics are concerned about the extent of her head and spinal injuries, so she needs to be urgently assessed. Diabetic David, 53, woke up struggling to breathe with a severely swollen throat, but with his condition worsening he urgently needs to be moved to resus.

Paramedics bring in a woman who has an incurable lung disease known as COPD and is struggling to breathe. Eighty-year-old Michael has walked in after he fell running for a bus and banged his head and goes to the top of Dr Chris Yeoman's list.

Sister Jane has started her shift and has just walked onto the floor to find that every single area of the department is full, with 90 patients in need of help from the casualty team.

As if it wasn't busy enough inside the department, there's now a problem outside too. Doctor Jake Mullen and a team of nurses have been alerted to an 86-year-old screaming in pain in the car park. Mary has fallen over landing badly on her hip and once she's placed onto a trolley is rushed straight to resus. Doctor Mullen must work quickly to find out whether her injuries could be life threatening.

Another emergency is on the way. Paramedics are blue-lighting in a 54-year-old who's had back-to back seizures at home. The last time this happened abscesses were found on Paul's brain. Although Paul now appears well, there are too many red flags. Nurse Aleena immediately asks Doctor Hridesh Chatha to assess him. If the abscesses have come back, Paul is in danger of needing emergency brain surgery.

On busy days like today, the team of volunteers like Jane, play a vital role in keeping the department moving. One of the most vital jobs the volunteers help with is keeping the patients who are staying the longest, fed and watered.

Over in Paediatrics, four-year-old Kian may have broken his thumb after a fall. His mum Becky is with him and waiting to find out what the damage is. Nurse Greg assesses him and orders an urgent x-ray.

And paramedics are bringing in an 80-year-old who's been struggling to breathe. Advanced Clinical Practitioner Chantelle heads to Cubicle 4. Patricia's oxygen levels are worryingly low, so she needs to quickly find the reason why.

It's Bank Holiday weekend, but for the team at Barnsley Casualty it's business as usual. They're bracing themselves for a chaotic day and in charge of the shift is Sister Jane alongside Consultant James Griffiths. To make matters worse Barnsley are playing a key football match and once it's over they expect the patient numbers to suddenly rise.

The first emergency is on its way: 52-year-old Maggie is being rushed in after a serious car crash. Doctor Griffiths started treating this patient before he even clocked on, as the accident happened near to where he lives. Maggie has the bone disease osteoporosis, and it's suspected that she's badly broken her leg.

Over in Minors is the latest addition to the Barnsley Casualty team: Emergency Nurse Practitioner Clare. Her next patient is 70-year-old Stephen, who's suffered a deep wound to his head after falling over while walking his neighbour's dog. Emergency Nurse Practitioner Clare needs to remove every piece of grit embedded in his skin to avoid the wound becoming infected.

Back in the Hub, Sister Jane is trying to coordinate a crowded department, while Doctor Griffiths is wading through a sea of patients. His next is 26-year-old Jordan who's hurt his hand while riding a motocross bike. If he has broken a bone, it could have a major effect on his livelihood.

In paediatrics is another casualty of a sunny weekend. Nine-year-old Harrison has come in with dad Nathan, after being badly sunburnt from a day at the beach. Advanced Nurse Practitioner Liam is on hand to look after him.

Sister Harriet is assisted by sisters Benita, Jane and Jo for the hottest day of the year so far, with both walk-in and ambulance patients queuing up rapidly. Paramedics blue-light in a 61-year-old, who's doubled over in pain, Dr Jake Mullen preps for his arrival. An ambulance brings in 90-year-old Hazel, who's fallen and may have damaged her hip and neck.

Sister Rachel is in charge of the hub, every patient should have an initial assessment, called a triage, within 15 minutes of arrival, but an influx of patients has meant the wait time is longer. A 67-year-old diabetic has dangerously high blood sugar levels, putting him at risk of a diabetic coma and five-year-old Seb is struggling to stay awake after a bump to his head.

There's a shortage of space on the specialist wards that admit casualty patients, meaning the department is at risk of complete gridlock. Brian's in terrible pain after a bad fall at work, so consultant Bharath Reddy comes in to assess the damage. An ambulance rushes in an elderly woman who has been vomiting blood, so Doctor Jake Mullen prepares resus for her arrival.

Physician Associate Adele oversees 97-year-old Jim's treatment, who has suspected multiple organ failure and an 83-year-old man suffering from severe abdominal pain is seen by junior Dr Josh Gemmill. In minor injuries, Anthony is rushed in by his partner Lisa, following an accident demolishing a bathroom, when a shard of porcelain sliced open his leg.

There's been an influx of patients overnight, creating a huge backlog in the department and there's another emergency on its way. A lorry has tipped over on its side in a country lane and the driver, Karl, is rushed in. An 82-year-old is rushed in after they were found collapsed at home and 28-year-old Kial and his partner Becky wait to be seen after he sliced his fingertips preparing lunch.

On a 'Manic Monday' at Barnsley Casualty, paramedics blue-light in a 66-year-old with a dangerously high heartrate. Doctor Jake Mullen heads to resus to prep for Howerd's arrival. Doctor Mullen has the charts from the paramedics, and it's not good reading. Elvin, 74, suffers from the opposite condition to Howerd, but it's just as dangerous. His heart is beating perilously slow, but he's been ignoring his symptoms until his daughter Jade found out.

Barnsley Hospital's casualty department is flooded as ambulances pour in at the start of the day shift. Sister Abbey must keep things moving quickly as patients arrive with a variety of urgent needs. John, a 78-yearold man, is suspected of having severe cellulitis and is also paralysed from the neck down. Stacey, 37, has a serious arm injury after being thrown from a horse. Toddler Esmae has a head wound, while electrician James faces possible fingertip amputation. And Doctor Jake Mullen treats former rugby player Jeremy's swollen knees, suspecting a joint infection.

On another busy day at Barnsley Casualty, Sister Jane faces staffing shortages after some nurses called in sick. Twenty-one-year-old Harry arrives, showing signs of sepsis from an infection. Doctor Alice Duffy and the team will need to act fast to find out where his infection is coming from. Delivery driver Ian waits for treatment after an eye injury. Young Isaac has had a stone stuck in his ear for three days. Doctor Alex Webster assesses 89-year-old Eric, who has a complex medical history and keeps collapsing.
